There's a new CDC form your vet has to submit

Alongside the USDA health certificate, your vet now completes a Certification of U.S.-Issued Rabies Vaccination and submits it to the USDA — it certifies that your dog's rabies vaccine was issued in the United States.

It is easy to sort out while you are still at home, and a headache once you are on the island. We found out about it the hard way. Ask your vet for it by name at the same appointment as your health certificate.
